Republic of the Philippines Provincial Government of Pampanga

Diosdado P. Macapagal Memorial Hospital

Guagua, Pampanga October 22, 2013

EDDIE G. PONIO, FICS, CESE Public Health Officer 1 Hospital Administrator

Good day! This letter is in regards with the elected position as a team leader as agreed upon last October 2, 2013 during the meeting of the 18. I am fully aware of the recommended additional duties and responsibilities bestowed upon me as I accepted the position as one of the team leaders of the new J.O. nurses. As I would like to bring change for the betterment of the nursing service and the hospital in general, I regret to inform you that I will be resigning from the position due to many reasons. As of now, I cannot take full responsibility from all the task assigned and designated to me due to number of workloads in the master degree program I am currently enrolled and at the same time workload here in the institution. I would not like to compromise my duty as a nurse in taking care of my patients and giving quality excellent care. Burnouts will affect my performance and moreover detrimental to my health. Currently, I am the elected president of the Master of Arts in Nursing/Masters in Nursing Graduate Program A.Y. 2013-2014. I aim to focus my attention first on finishing my MA degree. I am in my last academic semester on my post-graduate degree specifically Masters in Nursing in Angeles University Foundation. Since this is my last academic semester, there are many academic requirements to be passed and would entail a lot of preparation for my practicum as a final requirement to graduate to that said degree. Thank you for your understanding and kind consideration.

Arnold L. De Guzman Jr. RN J.O. Nurse
