Reaction: Developing countries, including the Philippines, still have high rates in the prevalence of glomerulonephritis. As seen in the screenshot above, although this is just an extrapolated statistics, it still indicate that our country is one of the countries that have a higher prevalence and incidence of certain diseases or conditions in the Southeastern Asia. This is also in comparison with the update on acute post infectious glomerulonephritis worldwide said in a journal last May 2009. It was said there that high rates still persist in some developing communities. I came across an article about Pediatric Nephrology: Nephrotic Syndrome. That article was brief but very informative. Included in the article was the definition, diagnosing procedure, its probable cause, complications, treatment / management and recommendations. It also mentioned here the reason (signs and symptoms) to contact the physician in case of complications. Its reference is updated and reliable since it came from National Kidney Foundation (2011). I find this article beneficial in giving health care education to my client. As my client will be discharged, I think it would be necessary if I can give them brief health teachings regarding what signs and symptoms would be alarming that needs prompt consultation. I would also emphasize to them the need for compliance to diet regimen and medications and also to follow-up consultations.

I have encountered an article from Advance for Nurses with the topic Pediatric Nephrotic Syndrome. It cited there a case study with its treatment and follow-up plans for a pediatric disorder. I found this article noteworthy since it mentioned there nursing related topics including its plan of care, issues such as family, age-related, cultural barriers, social concerns, and nutrition therapy that the nurse must expound during nurse-patient interaction. I was also able to read a journal from the Paediatrics and Child Health (2011) about Glomerulonephritides. It is defined that these are group of disorders characterized by intraglomerular inflammation, manifested clinically by hypertension, hematuria and edema. As these manifestations were all present in my case study, I became interested in this topic. It was indicated in the journal that appropriate assessment of every child is required to ensure more serious diagnoses are not overlooked. I agree with this statement because assessment is vital to the care of our patient since treatment mainly rely on the findings of our assessment and physical evaluation. Though treatment is largely supportive, we must look into the point that we strictly implement close monitoring and observation of the patient. In relation to my case study, the pediatric patient was closely monitored especially blood pressure, daily weight, and urine output. The client was responding to treatment at first but there came a time that consistently the urine output was decreasing. The client had oliguria, there was increase in the temperature and blood pressure, and manifested a seizure. Due to low immune system, the patient had overwhelming infection as evidenced by elevation of WBC. Referral and consultation to pediatric nephrologist was needed. When complications occur, prompt treatment is necessary. Untreated glomerulonephritis may lead to kidney failure. So, recognizing the glomerulonephritis symptoms, especially in children, is very important. As children may not be able to describe the symptoms correctly.
