CICO Sheet

This document appears to be a daily behavior tracking sheet for a student. It tracks the student's behavior during different periods of the school day and assigns points from 0-3 based on the number of reminders needed for positive behavior. Goals are listed at the top for being kind, respectful, and on task. The student and staff member sign at the bottom with space for comments. The percentage of total possible points earned is calculated to see if goals were met for the day.
