vocational and/or employment history. I further authorize any current/former employer, person, firm,
corporation, educational or vocational institution, or government agency to provide Tarrant City Schools
with information regarding me.
If employed, I agree to provide all necessary paperwork including, but not limited to, completed tax forms,
I-9 (Employment Eligibility Form), proof of negative T.B. test results, teaching certificate, copy of driver’s
license, copy of social security card or other acceptable documentation pertinent to the I-9. I understand
and agree that, if all necessary forms and documentation are not provided, I will not be paid until
such time that said forms and documents are completed and turned in to Personnel.
