Directions: This self-evaluation is designed to help you diagnose the areas in which you are lacking in
your preparedness plan. Most people think they are prepared but in reality, are not as ready for a disaster
or traumatic event as they have been mislead to believe. Use this evaluation to determine what steps you
need to take in completing your own preparedness plan.
Surviving “In-Place”

1. Do you have enough food stored for every member of your family to eat a nutritious meal 2x each
day for 1 month?
2. Do you have enough food stored for every member of your family to eat a nutritious meal 2x each
day for 3 months?
3. Do you have enough food stored for every member of your family to eat a nutritious meal 2x each

4. Do you have enough food stored for every member of your family to eat a nutritious meal 2x each

5. Do you know how to raise small livestock such as; chickens, rabbits, goats, etc?

power (a week or longer)?
14. Do you have a survival plan in place in the event of a nuclear disaster or attack?
15. Do you have a survival plan in place in the event of grid collapse?
16. Do you have a backup generator with enough fuel to run for at least 1 month?
17. Do you have a stockpile of necessary medicines (antibiotics, analgesics, prescription drugs, etc.)
and emergency medical supplies?

9. If you had to bug-out into the wilderness, could you build an expedient shelter out of natural mate-
rials that would adequately protect you from the elements for an extended period of time (1 week or
more)?
10. Do you know how to start a fire without matches or a lighter?
11. Do you live in a location that rarely experiences natural disasters, such as tornadoes, earthquakes
or hurricanes?
12. Are you free from dependence on substances such as; nicotine, alcohol, caffeine, pain killers,
prescription medication, oxygen?
13. Do you have a portable emergency medical kit (blow-out bag) which includes trauma supplies
like suture needles, medical gloves, tourniquet, bandages, clotting agents, etc.?
14. Do you know how to suture wounds, handle serious injuries or have medical training?
15. Do you have a shotgun and at least 1000 rounds of ammunition?
16. Do you have a rifle and at least 1000 rounds of ammunition?
