My patient is Jennifer, 15 years of age, and she was brought here at Ospital ng Sampaloc because she had a Normal delivery to a healthy baby boy last Jan 30, 2012. She is currently living with her parents and with her live in partner as well. She is the only child. Jennifer had finished 1st year high school at Dr. Josefa Jara Martinez High School at Tatalon , Quezon City, She also said that she had to stop school due to her pregnancy. But she is planning to go back to school on the next semester. Her hobby is singing. For financial support she and her live in partner are being supported by their both parents. Her family has a history of hypertension and diabetes. On her daily living she loves to watch the noon time shows on the television, she particularly referring to Show Time and Eat bulaga, and other noon time teleserye. And during her pregnancy she only completed two prenatal check-ups and had not completed her vaccinations. Every Sunday she and her family goes to the church.
