RE : Added information pertinent to the practicum of the students

FROM : Unit Head

1. Please take note that the activities the students are undertaking
during the practicum in your plant/establishment is subject to your
instructional/management policies. Please feel free to give the proper
responsibilities and to decide when and how long they will be doing their
practicum in your food plant/establishment.

2. The students are instructed to have their field notebook with them
to record the daily activities undertaken after which the students will
request you to affix your signature every now and then.

3. Any misconduct the students make during the practicum should be


reflected in the performance evaluation supplied. Please use additional
sheets in item B of the performance evaluation form if necessary.

4. The evaluation is confidential which will be sent to Xavier


University, College of Agriculture with your certification of satisfactory
performance. If you have your own way of evaluating the performance of
the students, Please feel free to do so.
