Pakyaw Agreement

PAKYAW AGREEMENT

KNOW ALL MEN BY THIS PRESENT:

This agreement made and entered into this 8th day of November, 2010 at Tarragona, Davao
Oriental, represented herewith by the School Principal duly authorized for the purpose and them after
referred to as the implementing Agency and Arnel C. Manilong, private contractor of Tarragona,
Davao Oriental for the Repair and Replacement of ceiling and ceiling boards.

WITNESSETH

That in consideration of the rates in the program of works to this agreement and made part
hereof the parties covenant and agrees as follows:
1. That the Pakyaw Contractor of the rates in the Program of Works to this agreement for the
proper and faithful performance as described below:
LUMPSUM…………………………..P10, 325.00
2. That the Pakyaw Contractor shall complete the work in accordance with the corresponding
plans and specification under the supervision of the School Property Custodian and Physical
Facilities coordinator designated by the School Principal.
3. That the Pakyaw Contractor shall further comply with the terms and conditions of this contract
especially in the attached Program of Work.
4. That the Pakyaw Contractor agree to commence the work within ten (10) days upon signing of
this contract and complete the same in ten (10) days, Sundays and legal holidays included.
5. That the payment will be based on the measured actual accomplishment as stated in the
Program of Work and accepted by the implementing agency.

In witness hereof the parties aforesaid have hereto affixed their signatures on the first above
