What is Dengue?

an infectious disease caused by a virus from an infected


mosquito, which is passed on to a person. It is common in
warm, wet areas of the world and outbreaks tend to occur
in the rainy season.
Swahili phrase “KA-DINGA PEPO”
•Disease caused by an evil spirit
•DINGA = Spanish (fastidious/careful)
= gait of a person suffering the bone pain of
dengue fever
Synonyms:
Breakbone Fever
Dandy Fever
Thrombocytopenic Purpura

Benign – Dengue Fever


Malignant – Dengue Hemorrhagic Fever
Mode of Transmission

•Bite of infected female Aedes Aegypti or


Aedes Albopictus
•Aedes aegypti mosquito
-Day biting (2 hour before sunrise and
2 hour after sunset)
-Low flying
-Lives in stagnant water
-Common in urban with white spots at
the base of the wings and white bands
in the legs
-Ordinary mosquito
Incubation period: 6-7 days

Diagnostic Tests:
•Tourniquet/ Rumpel Leede Capillary Fragility Test
(presumptive)
-A blood pressure cuff is applied and inflated to a
point between the systolic and diastolic blood pressure
for five minutes. The test is positive if there are more
than 20 petechiae per square inch (a petechiae is a
small red or purple spot on the body, caused by a
minor haemorrhage).

•Platelet Count = 150000-400000mm3

•Hematocrit Count = 20%

WITH SHOCK: IVF CRYSTALLOIDS + PLR/PNSS
20ML/KBW IV IN LESS THAN 20 MINS
NO IMPROVEMENT: COLLOIDS (DEXTRAN)
10ML/KBW IN 40 MINS
STILL NO IMPROVEMENT: FRESH FROZEN PLASMA
15ML/KBW IN 2HRS.
