EYE SWABBING

PURPOSES:
1. To clean an infected eye
2. To clean eye from crust or discharge.
3. To provide comfort to client with eye problem
4. To clean eye before instilling eye drops/eye ointment

ACTION RATIONAL
ASSESSMENT

1. Assess client’s condition To identify client able to follow


I. General condition instruction and co-operate with
nurses.

II. Eye
 Discharge (hipopion) To identify any abnormalities of
 Redness/swelling of eye client’s eye
 Bleeding in the eye
 Operation site and suture
 Eye movement
 Structure of external eye.

III. Patient complaint


 Painful Provide baseline and identify
 Itching contraindication for this procedure

IV. Environment. To provide adequate working place

PLANNING

Prepare the equipment:

1. Eye dressing set.


a. Kidney dish
b. Gallipot
2. Normal saline
3. Swab
4. Receiver
5. Tray

Get help from staff if needed.


IMPLEMENTATION
1. Introduce self, greet client and explain the To gain co-operation from client

2. Assist the patient into the correct position: head To identify suitable position of the

II. Swab the eye from inner chanthus to the Cleansing from inner to outer

III. Swab the upper lid( instruct patient to look


down)
IV. Swab the lower lid(instruct patient look up
the ceiling)
V. Swab the middle lid(close the eye)
VI. Using a new swab each time; repeat the
procedure until all the undesired material
has been removed

7. Repeat step 6 for the another eye

8. Once both eyelids have been cleansed and dried,


make the patient comfortable

9. Remove and dispose of equipment

10. Wash hands

 For infectious Eyes: the affected eye is cleaned last to prevent spread of infection to the
