Book Report Form1

Dear Parents & Family,


Please read today’s library book together, taking good care of the book. Have your child point out the front cover,
back cover, and spine of the book. If your child is able to, have him/her write his/her name on the book report form
for practice. The adult will do the rest of the writing. It is optional for your child to draw a picture on the back of this
paper. Enjoy this special time together! Return both the library book & this form in the Homework Folder
tomorrow.
