AFFIDAVIT
I, NATHANIEL P. CASUNCAD, of legal age, Filipino citizen, married, and
a resident of 52-E Calumpit St., Binictican, Subic Bay Freeport Zone, after
having been duly sworn in accordance with law, hereby depose and say:
1. That I am presently employed to the SBMAs Corporate Secretarys
Office, Subic Bay Freeport Zone, and by reason thereof, was issued an
SBMA Identification Card No. 796163987;
2. That last September 6, 2011 when I reached our home, I inspected my
things, and I found out that the SBMA ID issued to me was lost and
nowhere to be found;
3. That I am executing this affidavit to attest to the truth of the foregoing,
to request for the replacement of my lost SBMA ID, and for whatever
legal purpose it may serve.
Further Affiant sayeth none.
IN WITNESS WHEREOF, I have hereunto affixed my signature this 9 th
day of September 2011 in Olongapo City, Philippines.
