The College of Nurses of Ontario (the College) collects the information in the Learning Plan for Practice Assessment under the general authority of the Regulated
Health Professions Act, 1991, S.O. 1991, c. 18, the Nursing Act, 1991, S.O. 1991, c. 32, and its regulations, and the Colleges by-laws. The College collects the
information for the purpose of assessing your continuing competence through its Quality Assurance Program. Appropriate measures are taken to safeguard the
confidentiality of the personal information you provide, and all documents become the property of the College.

What learning needs did I identify through Practice Reflection?
The learning needs I have identified through Practice Reflection are to do further research and practice on
assessments and other crucial aspects of nursing care, to be more efficient with time, and to think critically in
certain situations especially before medication administration to prevent complications and harm to the patient's
health. The strengths I have identified in my nursing practice are: 1) I quickly perform documentation and I have

gotten used to the system and also gotten used to writing nursing notes. 2) I am always eager to learn and open
minded to new learning opportunities. 3) I keep communication open to my patient even though there is a barrier
in language.

What do I want to learn?
Which practice document does my goal relate to?
What I want to learn is to be more efficient with time and care that I am providing to my
patient in order to help prepare myself in the future. I notice that I would sometimes
be behind in patient care when comparing to my other peers so I want to change this
part of my nursing practice and improve it.

Goal #1: Activities and Timeframes


How am I going to achieve my goal?
I am going to achieve this goal by using my time wisely, take initiative and become more independent and confident with patient care as
this will prevent and avoid delay with patient care. Although there are times where medication administration in the morning occurs with
my other peers and there is time conflict present since my teacher has to evaluate each student with medication administration, there is no
excuse to not give other means of care to my patient such as giving a bed bath or assisting them to the toilet or take vitals and etc. So this
is where I should take more initiative of my care to the patient to prevent and avoid further delay to the care of my patient.

What do I want to learn?
Which practice document does my goal relate to?
What I want to learn is how to properly take health assessments of my patient in order
to identify abnormalities that must be taken care of immediately to promote the health
of my patient. There are times where I do not perform health assessments due to conflict

How am I going to achieve my goal?
I am going to achieve this goal by attending to open access labs with peers to practice the different health assessments in which they can
later provide useful feedback based on my assessments. Also, I am going to research and reread the checklists from last semester to
enhance my knowledge with the different assessments to identify abnormalities that may be present in my patient. Lastly, in order to
actually complete a health assessment on my patient, I am going to perform the health assessments in the morning, mainly during hygiene
care. In this case, everything will be taken care of before the patient goes to therapy or has family visiting them.

What do I want to learn?
Which practice document does my goal relate to?
What I want to learn is how to make better decisions and avoid complications by thinking
critically of how to take care of my patient in certain situations. This applies to medication
administration as I should know the different contraindications and precautions that comes
with each drug, knowing so will prevent harm to my patient's health.

Goal #3: Activities and Timeframes


How am I going to achieve my goal?
I am going to achieve this goal by doing research and implementing my knowledge from theory and pharmacology to real life situations
as situations you'd never expect to happen will most likely occur; basically, I should prepare myself with the knowledge so I can expect
the unexpected. When giving medication administration, knowledge is crucial related to pharmacology as the mechanism of action of
certain drugs should be acknowledged. Doing so will promote patient's health as he/she is out of harm's way; I will definitely review
and do further research of the drugs needed to administer.

My learning relates to the competencies of my practice because it shows if my nursing
practice has improved over the course of time it took to write this assessment to the next
assessment I will write.
