Learning Goal #1
My goal must be based on my learning needs.
Collaborate and consult with clients and the interprofessional and health care teams
to provide comprehensive and holistic health care in a complex and changing
environment.
Although I have done this with the assistance of my preceptor, I want to be able to
do this regularly and to practice this skill so that I feel so self-assured and secure
when collaborating with doctors.

Be more assertive, not feel afraid to talk to the doctors. First get to know the doctors of the unit so that when I actually have a
patient that they are responsible for, I will know who or how I should approach them. (Start practicing at home, after exams)
Be friendly and approachable (usually when there is open, and unconflicted relationship, there is less tension) (This should be
automatic no timeframe needed)
I will use effective communication skills:
(Start practicing at home, after exams and will continue throughout career)
a. Knowing relevant information (of the pt, the situation, and why I need the collaboration at this time)
b. Always do my assessment first, so that I know what is going on and what to tell the doctor
c. Talk clearly, concisely, and use a system such as SBAR so that no ones time is wasted
Research for some training and seminars that are provided to help with communicating with other health care providers e.g.
nurse to doctors, nurse to other teams of the interprofessional groups.

Learning Goal #2
My goal must be based on my learning needs.
Practice to have the clinical eye and have a more detailed head-to-toe assessment.
I observed the nurses whenever the shift starts or when we have a post-op coming in,
the head to toe assessment is performed, for me as a newbie, I have to have a
checklist for myself to be able to remember the head-to-toe assessment and that I do
not miss anything. For them, just inspecting the patient they already know where to
focus on and where to do just a basic assessment.
Go back to my health assessment textbook, and to help prepare for my licensure exam I will do a different system per session
so that I will have that check-list memorized and ingrained in my brain. (Studying towards the license exam and thereafter)
The cue-cards that I made for health assessment will be very helpful and so will other health assessment checklists from other
sources (nursing journals, internet, textbooks e.g.) (Studying towards the license exam and thereafter)

Learning Goal #3
My goal must be based on my learning needs.
Focus on a healthier lifestyle, exercise more, loose weight.
I need to do this lifestyle change so that I can last a long time in this very mentally and
physically demanding profession.

Truly watching my diet (Start now, and follow-through to get results and keep on doing it to keep the desired result)
a. Use calorie counting to keep my caloric intake at a regular (2000 cal/day) for a health weight management
b. Watch what I eat less fat and junkfood, more vegetables, and lean protein
c. Limit myself to 1 cheatday per week, as supposed to now that I sometimes eat out at up to 4 times a week with my
family
d. Drink more water
Exercise at least 4 times a week for at least 30minutes of physical activity
Set time for relaxation independently, and set time for family bonding to keep happy relationships (At least 30min per day)
