Dear Sir/Madam,
LOST OF COMPANYS ACCESS CARD
As above matter I would like to inform YKRC management that I had lost my access card.

During the first month of internship in YKRC I was still using my access card,
however after that I get to realized that my access card is only can be accessed the main door
of YKRC. So I decided just to leave it at home in order to prevent any lost during my journey
from my house to YKRC.

As Im going to finish my internship soon on 15th April 2016, I decided to find the
access card back so that I can return in back to YKRC. I had tried to find it everywhere in my
house and YKRC, however I couldnt find it and I had come to a conclusion that Ive really
lost my access card. Even recently during the month of March I had brought to YKRC few
times.

As a trainee I understand that loosing companys access card is a serious issues which
could cause security problem. I really would like to apologize on the issues as it happened
because of my carelessness. As in return Im ready to pay the penalty for the loss of access
card.
I hope that YKRC management can accept my deepest apologies for my mistake in regard to
losing the access card.
