LETTER OF CONSENT SURAT PERSETUJUAN

I hereby submit my resume to Prumatech (M) Sdn Bhd (Company No. 86445-T). I have been notified
that my personal details shall be processed by Prumatech (M) Sdn Bhd and its business partners or
parties that it contracts with ExxonMobil Exploration and Production Malaysia Inc. during the course
of my service with Prumatech (M) Sdn Bhd and I hereby consent to such processing.
Saya dengan ini menyerahkan resume saya kepada Prumatech (M) Sdn Bhd (No. Syarikat 86445T).Saya telah dimaklumkan bahawa butiran peribadi saya akan diproses oleh Prumatech (M) Sdn Bhd
dan rakan-rakan niaganya atau pihak yang berkontrak dengannya ExxonMobil Exploration and
Production Malaysia Inc. sepanjang tempoh perkhidmatan saya dengan Prumatech (M) Sdn Bhd dan
saya dengan ini bersetuju dengan pemprosesan tersebut.
The Daily Rate due to me shall be RM________/day, which shall comprise of the Base Rate of
RM______/ day and the premium of 30% / 10% / 0%, applicable to the base work location to which I
shall be assigned to, that is, (please select the applicable location :) offshore/onshore/KL office.
Kadar Bayaran Harian yang akan saya terima adalah berjumlah RM_______ / sehari, yang meliputi
Kadar Bayaran Asas berjumlah RM____________ / sehari bersama dengan kadar premium ______,
tertakluk kepada lokasi kerja asas yang telah ditetapkan keatas saya, iaitu (sila pilih yang sesuai :)
offshore/onshore/pejabat KL.
I understand that the Base Rate shall be solely based on my qualification and work experience and is
inclusive of any allowances and benefits, monetary or otherwise, but excludes any payroll burden
imposed on my employer (e.g. employers contribution to EPF, SOCSO, and medical insurance). Such
Base Rate shall then be used to calculate the applicable Daily Rate that I will be compensated with,
as and when there is a change to the base work location that I am assigned to.
Saya dengan ini memahami bahawa Kadar Bayaran Asas yang dinyatakan adalah semata-mata
berdasarkan kepada kelayakan dan pengalaman kerja saya, termasuk apa-apa elaun atau faedah,
samada dari segi kewangan atau sebaliknya, tetapi tidak termasuk mana-mana beban gaji yang
dikenakan keatas majikan saya (contohnya sumbangan majikan kepada KWSP, PERKESO, dan
insurans perubatan). Kadar Bayaran Asas tersebut akan digunakan untuk mengira Kadar Bayaran
Harian yang akan dibayar kepada saya, apabila terdapat perubahan kepada lokasi kerja asas
dimana saya akan ditugaskan.
