General Information:
John Doe was born on 5-10-72 in Mitchel. He cannot make informed decisions
alone so he is in need of a guardian. His current guardians are Lois (Mother)
and Marcus (Brother) Stacey.
He currently lives at 111 123 Street, which is a group home in Madison and
his phone number is (***) ***-****. He has been in the ECCO system since
1993. John Doe has no major health concerns. However, he doesnt always
tell people if he is in pain. John Doe doesnt need assistance to walk. He is
mainly independent but may need reminders to wash hands and flush the
toilet.
To communicate with John Doe, speak clearly and avoid verbal cues. If John
Doe begins to roam or attempt to leave it is usually a sign that he is getting
anxious. He receives 32 hour of pre-vocational services weekly and works at
the recycling every day. He doesnt like being asked questions when the
answers are obvious, pushy demanding people, too many choice, and not
having snacks.

Strengths, Interests, and Needs


John Doe enjoys being outside, cooking, going to movies, playing games
such as Uno or the Wii, and puzzles. John Doe has worked at the same place

for quite some time so he is demonstrating dependability. He is mainly


independent. He needs supervision with cooking especially the preparation
portion. He needs supervision while outside because he has a record of
trying to run away. He also needs supervision when playing the Wii because
he tries to put more than one disk in at a time.

History
John Doe was in a childrens facility until he was referred to ECCO. He
currently works at the recycling center and the ECCO workshop. His dad was
a welder. He passed away in 2011 and John Doe attended the funeral. His
mom worked in a public Library in South Dakota. His parents were divorced.
He has two brothers and two sisters and he is the middle child. He is satisfied
with his family contact and goes home on holidays.

Medical History
He is diagnosed with mild autism and mild cognitive impairments. His
general health is good. Only medication he takes is to relieve constipation.
John Doe wore glasses but now doesnt need them. In 2000, he got frostbite
on his hands from riding his bike in the winter to his grandmas house across
town.

Logistics
John Doe is generally independent in walking, eating, clothing, and
bathroom. He needs assistance with meal preparation because he has a
tendency to eat raw or frozen food in large quantities. John Doe has a food
restriction because of unsafe cooking methods.
John Doe has a tendency to damage Wii games and only has access to one
disc at a time. He likes to put more than one disk in at the same time so, his
games are left in a locked cabinet.
He struggles with expressive communication and doesnt always verbalize
his wants and needs. When he does verbalize them he does it well. He
responds best to written directions or cues and doesnt respond well to
verbal cues. John Doe struggles with understanding slang or teasing and
takes everything literally. John Doe has to be with someone when walking
outside because he tries to run away. However, he hasnt tried to run away
since 2012.

Educational
John Doe attended special education classes through 6th grade, during that
time he was in a self-contained classroom but left for P.E. and art. He lived at

home during this time. His peers were good with him until the middle of 6th
grade. There was only one special education teacher in his district and she
wasnt equipped to handle his needs. So, he went to Vermillion ATC. His
mother requested he be placed in Yankton. He remained in Yankton for only a
year because he had a negative experience. This is where he started to run
away. During his stay in Yankton, he ran away and started a fire in a church
with his brother. After that he moved to the Black Hills Special Services Coop.
He ran away several times here. He was then referred to ECCO because he
was turning 21.

Preference Assessment Reflection

Overall, the preference assessment went better than we planned.


When we arrived for the day, John Doe was putting together a 1,000 piece
puzzle in the living room of his group home. We had him come to the kitchen
table to perform the assessment. Trial one went well. When asked to identify
between the two items John Doe would usually pick up the item and shake it
rather than use his words to verbally communicate to us which one he
preferred. When he had to choose between two items such as solitaire or the
Mario Brothers Wii game he would hold both of them up and it took him a
while to pick one because he really liked them both. When he saw the picture
of the puzzle towards the end of Trial 1, John Doe would stand up and try to
go back over to the puzzle he was working on. Therefore, we took a break
after trial one and worked on the puzzle with John Doe.
Trial 2 was a bit more challenging. John Doe was getting anxious and
was picking between two of them more nonchalantly trying to get done. We
kept telling him only five more or only two more to keep him in his seat.
Overall, John Does top three choices were Solitaire, Puzzle, and the Mario
Brothers Wii game. We had suspected this to be the end result based on our
first visit with John Doe the previous week.

John Does Goals and Objectives

Goal:
Prior to calling his mother, John Doe will type a list of what he wants to say on the
computer with less than three verbal prompts with 90% accuracy in 8 out of 10
trials.

Objective:
1. John Doe will copy written words from paper to a word document on the
computer.
2. John Doe will create his own list of 5 or more words.
3. John Doe will type his list of words with fewer prompts.

We did a task analysis of our goals and found that John Doe possesses typing skills
already. His computer is not equipped with the proper applications to continue to
build on his typing skills. John Doe can type independently if someone gives him the
words to write. We will continue to work on typing skills, but our main focus will be
expanding basic computer skills and application knowledge. 9/30/15

Overall, John Does computer skills were about what we expected. He does
have experience using a desktop computer, but not a laptop. John Doe got bored
quickly when using word. He did not like having to type the list, and wouldnt come
up with words on his own, even with verbal prompts. His biggest struggle was using
the mousepad on the laptop. He is use to using a physical mouse. When playing the
games, we showed John Doe how to use the touch screen instead of the mouse,
which made it a little easier. John Doe would drag his finger across the screen
instead of clicking. He needed verbal prompts for everything during the task
analysis. The only thing we had to model/physically do involved using the mouse
and finding the windows key for the first time. He wanted to continue playing the
games after the analysis. John Doe caught on really quickly, especially with PacMan. By the end he needed very few prompts.

Overall, the lesson went very well. John Doe showed progress from the first trial to
the fifth. 3 out of the 5 trials are unsuccessful due to excessive prompts. Trials 4 and
5 both only required 3 prompts, making them successful. John Doe was getting
frustrated toward the end of trial 3 with opening and closing the computer so much.
We took a break and let him choose between solitaire and the puzzle. John Doe
decided to play bejeweled. We played for about 5 minutes until he was ready to
practice opening the computer, logging in, opening the word document, closing the
document, and shutting the laptop. The trials after the break went very well. We
learned that John Doe responds well to numbers and visual ques. For the next
lesson, we will tell him how many trials practices we will do, and count them down
with him so he knows where we are at.

At the beginning of the lesson today, John Doe seemed a little disheveled. He
improved as the lesson went on though. It seems like he is getting more
comfortable with us. John Doe did better with only clicking the mouse one time, with
the index finger. He used the arrow of the mouse to track where the Pac-Man was
going. He also was talking a lot more without being prompted to. When we asked,
what does the green button tell us to do? He responded with Green tells us to
keep on playing, so we will keep playing! Wanted to keep playing after the 5 trials,
so we didnt need a break or reinforcement. John Doe successfully opened and
played Pac- Man with three or fewer prompts three times, trials 3, 4, and 5. Trials 1
and 2 were unsuccessful because it John Doe needed more than three prompts.

John Doe opened the computer and logged in with very minimal prompts. He did
very well typing the words. He chicken pecks (types with his index fingers) but types
pretty fast and accurate. John Doe needed to be reminded to hit the space bar after
each word, and eventually put spaces without being reminded, but put several extra

spaces between words. The prompts that we gave were nonverbal (pointing). When
asked if he wanted to take a break he said yes, and that he wanted to play PacMan? He also started talking a lot more as the lesson went on. Next time, we may
put a piece of tape on the space bar to remind him to push it. John Doe successfully
wrote 6 out of 10 sentence with three or fewer prompts. The other 4 sentences
required more than 3 prompts.

John Doe did very well today. He did a good job of clicking once on each of the
jewels. He was also very talkative today. He would say hmmmm when thinking of
which jewel to click. A lot of the time, we would have to point to what jewels to click,
but he would sometimes do it on his own. John Doe wanted to play the minion game
on the computer as a reinforcement/break. It was pretty challenging so we played
solitaire instead.

was the first time it snowed, so that may have had something to do with it. He
would furrow his brow when we told him to switch from one of the games to word.

John Doe was in good mood today. He didnt need any prompts to open and login to
the computer today! His typing was fast and accurate. All we had to say was now
lets play Pac-Man, or now lets right a sentence. He sometimes needed
encouragement with the spelling of words. Judge Judy was on TV today so he was
really distracted. He laughed for the first time with us today. During Pac- Man, he
would say uh oh, the green one! He successfully completed 3 out of 4 trials today!

Today was a good day. John Doe finally remembered his password for the computer
and didnt need any prompts to get it started. He remembered to push the windows
key after playing bejeweled too. He also pushed the space bar without prompting
today. He was pretty talkative as well. Today was our most successful lesson, with

John Doe has exceeded our goals and expectations several times throughout
the course of the semester. Our initial goal was solely learning how to type. John
Doe quickly showed us that he has exceptional keyboarding and spelling skills. We
adjusted his goals to include broadening his computer skills and application
knowledge. Our main point of evaluation was how many prompts John Doe needed
to complete the tasks. The above graphs show a steady decline in the number of
prompts needed for each application and in each lesson. John Doe showed constant
progress in his learning. In each individual lesson, John Doe decreased the number
of prompts needed. This shows us that John Doe learned very quickly, and
maintained the knowledge. There were very few barriers to our progress. John Doe
was compliant and eager to learn every day. The biggest problem we had was with
scheduling. It would have been ideal if we could have gone more often, but all three
of our schedules prohibited it. It did work well going to John Does home at 4:00 pm
every Monday and Wednesday. It was right after he finished work and right before
supper time. John Doe progressed much faster and with much more success than
we initially expected. He began to get more comfortable and open with us as the
days went on, which really helped with our instruction and John Does learning.
Every day, John Doe showed improvement. There were several unexpected benefits,
we noticed, from the lessons as well. For example, John Does mouse skills improved
dramatically. His response time while playing Pac-Man decreased and it seemed like
his hand eye coordination improved. If we could continue with the lessons we would
add more applications, practice internet skills, and use the mouse pad on the laptop
instead of a wireless mouse. I would keep a similar lesson format to what we have
been doing, just adding more content each day. On a daily basis we used a Fujitsu

Laptop, wireless mouse, and several applications on the computer. We recommend


that John Doe upgrade his desktop computer to a portable Laptop and have access
to the internet. This will help John Doe continue to improve on his computer skills.
John Doe showed so much progress through the course of our time with him. With
continued instructions, his learning could be endless.
Assistive Technology Report
We believe that John Doe would benefit from an updated computer and
internet access. In the world of 2015, technology is everywhere and at the palm of
our hands exposing us to an open door of opportunities. Internet access is
something many of us take for granted. At the group home, we observed that there
is not a lot of technology for the residents to use. However, they did have cable TV
and a Wii. Here are our recommendations for assistive technology that may benefit
John Doe.
The computer we believe is best for John Doe can be found at Best Buy for
about $400. It is a 15.6 touch-screen Toshiba laptop. It has an Intel Core i3
processor. An Intel Core i7 processor would be faster, however, for what John Doe
uses it for the i3 would work just fine. He would also be saving money that way. It
has 6GB memory and a 1 terabyte hard drive, which would increase the amount of
storage. Windows 10 is the operating system that comes with the computer. This is
similar to Windows 8, which is what John Doe has been using on our laptops. They
both contain the windows key that brings up the tile start screen. This screen allows
him to pin his most frequent apps for easier access. Apps such as Microsoft word for
typing and games such as solitaire and minion rush can be easily be pinned to the
titled start screen. The tiles can then be easily formatted from small to large tiles.

The Toshiba laptop contains a built-in webcam. With this feature, John Doe
could skype his mom more often. The built-in stereo speakers will allow John Doe to
feel like he is a part of the game his is playing. It will make the games more
engaging. There is also a headphone jack so, John Doe could plug in headphones to
avoid bothering the other people who live with him. The lithium-ion battery is lighter
and charges faster than the regular battery. The lithium-ion battery also holds its
charge a lot longer to allow more time to play games. Overall, this computer was
rated a 3.4 out of five stars by Best Buy Costumers. Most stated it was a great
computer for the price paid.
Another recommendation is for John Doe to have a wireless mouse instead of
using the mousepad. The mouse is easier for him to navigate and he is used to
using one. A wireless mouse can be purchased at Wal-Mart for about $20. There are
plenty of designs and styles to choose from to find a perfect match.
A final recommendation we have is for John Doe to have internet access.
Internet costs about $35 per month at Midcontinent. We believe this would benefit
John Doe because he would be able to download different games with assistance.
This would also allow John Doe to skype his family and work on his communication
skills. He could also play games with people across the world and work on his handeye coordination. He could use the internet to watch #360 Videos on YouTube.
These are videos that allow you to go on adventures without even leaving your
house. John Doe could explore shipwrecks and underwater discoveries using the
#360 videos on YouTube. These videos allow people to experience adventures that
may not be possible to do in person.

In conclusion, our recommendations of assistive technology for John Doe is to


have an updated computer, a wireless mouse, and internet access. We believe that
these would benefit the overall quality of John Does everyday life.
