Letter of Intent

Ernesto and Shirley Bagtas are requesting permission for their son Aldwin Bagtas to accompany them to the airport from 7:00 am to 12:00 pm on October 16, 2015. Aldwin is scheduled to be on duty from 7:00 am until the next day with a 'once relieved' status, allowing him to leave once relieved of his duties. The Bagtas hope this time off will not interfere with Aldwin continuing to learn skills in his OB-GYNE rotation for the rest of the month.

To whom it may concern,


Warm Greetings with peace!
We, Ernesto A. Bagtas and Shirley D. Bagtas, parents of Aldwin D. Bagtas, would kindly
ask for your kind permission to allow our son ample time of accompanying us to the airport in
NAIA Terminal 3 today from 7:00 am onwards. We were told that his duty post marks from the
7:00 am shift until the next day as once relieved status. From this, he will be back to his duty
post not later than past 12:00 pm today.
We hope he would still cope up with skills and additional learnings in your department
OB-GYNE rotation in the coming duties until the end of the month. Thank you for your
consideration!
