Various sources can be used including the


experience from my mentor and self. I would
need to research the illnesses more common
treated within the community. I can use the
statistics of common diseases and treatments
that have been used based on evidence. I
would continue to use my text for the class in
order to learn more about the vulnerable
population that I will be caring for.

I was able to provide the community with evidenced based


practice on how to manage diabetes. Many of the patients that
visit the Care-A-Van had difficulty managing diabetes at home. I
was able to provide them with resources on diabetes such as
healthy plate, blood glucose logs to monitor at home, references
on blood glucose ranges.
