Plannedabsencecontract 2015-2016

This Planned Absence Contract outlines the process for a student to request an excused absence from school. The learning coach must submit the signed form to the teacher at least one week before the planned dates of absence. The contract specifies the reason for absence such as religious holidays, medical leave, family trips, or other planned events. It notes that vacations during testing times will not be approved. The learning coach acknowledges they understand the school may deny the request and enforcement of late work policies if the absence is not approved. They also understand the student will have two school days for every day absent to make up missed work.
