Dear Candidate:
As cohort coordinator, I am writing to provide an Initial Letter of Notice related to your
professional teacher candidate behavior. As we discussed in our conference, I am
concerned about your:
Repeated tardiness to and absences from your UHM classes and your field
experience placement.
Failure to notify your mentor teacher and field supervisor in advance of being late
or absent in your field placement.
Please take immediate steps to correct these problems. I will support your efforts by
helping you build time management strategies and other support systems to ensure your
success. I am confident that you can demonstrate the professional knowledge, skills,
and disposition required of teacher candidates in the Exceptional Student and
Elementary Education Program.
If you are unable to show progress in resolving these issues within two weeks, our next
step is to initiate a formal, written Plan of Assistance for Improvement that provides more
specific requirements and direction for improvement. Please contact me within two days
of receiving this letter to make an appointment to discuss your plans and progress.

Candidate has been very busy with her cohort classes and a full-time job. Her field
supervisors and mentors are concerned that she wont have enough time to plan,
prepare, and be fully engaged in student teaching. To assist, Candidates field supervisor
and mentor teacher will talk with her weekly to ensure she is meeting expectations for
attendance, planning, and engagement. In addition, her cohort coordinator will discuss
options for obtaining financial support during student teaching. The attached chart
contains expectations for improvement, teacher candidate actions, a timeline, and
program supports.
Next steps: This plan will not remain in the Candidates permanent file if Candidate is
able to meet specified expectations. If not, she will be required to repeat student
teaching, with a Plan of Assistance for Improvement in Place, or be dismissed from the
program.
Documentation of Understanding
I have read this plan and understand that my immediate attention to these areas of need
is critical for successful completion of my teacher education program. I understand that I
can respond in writing to this plan.
