Examination Application Form (NAPOLCOM Form 1-A) which may be


downloaded from the NAPOLCOM website (www.napolcom.gov.ph) duly
accomplished by the applicant.

2.

Two (2) recent and identical 1 in. x 1 in. color ID pictures (with white
background) and complete name tag (indicate rank before the first, middle
and last names).

3.

Examination Fee: Four Hundred Pesos (Php 400.00) for Police Officer, Four
Hundred Fifty Pesos (Php 450.00) for Senior Police Officer, Five Hundred
Pesos (Php 500.00) for Police Inspector and Six Hundred Pesos (Php
600.00) for Police Superintedent Exam.

4.

One (1) legal-size window envelope with fifteen pesos (Php 15.00) worth of
mailing stamp/s.

5.

Certified True Copy of Attested Appointment, Plantilla Appointment or PNP


Absortion Order.

6.

Report of Rating/Certification issued by the NAPOLCOM CENTRAL Office,


CSC Certificate of Eligibility and/or PRC Board Certificate.
