which the pupil is leaving
2. Name of the pupil
3. Name of the Father
4. Nationality, religion or caste
5. Date of birth (in words) as enterd in the admission
register
6. (a) Class or form in which the pupil was studying
at the time of leaving (in words)
(b) In the case of pupils of the higher forms
(i) Course offered, i.e., whether academic or
bifurcated and the subject offered under the
bifurcated course
(ii) Languages studied under Part I and Part II
in stds. VIII to XI as the case may be
(iii) School to which the pupil intends to join
7. DAte of admission or promotion to that class or
form (the year to be entered in words)
8. Whether qualified for promotion to a higher class
or form under the Madras Educational Rules and
the Secondary School Leaving Certificate rules
9. Whether the pupil has paid all the fees due to the school
(a) Whether the pupil was in receipt of any
scholarship (Nature of the Scholarship to be specified)
10. Whether the pupil has undergone medical inspection
during the year first or repeat (to e specified)
11. Date on which the pupil actually left the school
12. Date on which application for transfer certificate
was made on behalf of the pupil by his parent or
guardian
13. Date of the Transfer certificate
14. Reason for transfer
15. Signature of Headmaster / Headmistress
