Crash Reporting Guide

In the event of a car crash: 1. Stop and call 911 if needed. Move your vehicle off the road if possible to avoid blocking traffic. 2. Get the names, addresses, driver's license numbers, insurance information, and license plate numbers of all people involved and any witnesses. 3. Do not move any injured people unless necessary. Exchange the same information you collected with the other driver(s) involved.

In Case a Crash Occurs:


STOP. Cal l 9- 1- 1, i f needed.
I f you can, move your vehi cl e off
the road so that you do not bl ock
traff i c.
Get the names and addresses of
al l peopl e i nvol ved i n the crash
and any wi t nesses.
Do not move an i nj ured person
unl ess necessary.


Give the following information to the driver of the other vehicle
and request it in return:
Name and address. Damage t o vehi cl e.
Dri ver l i cense number and stat e
of i ssue.
I nsurance company name.
Vehi cl e l i cense pl at e number and
stat e of i ssue.
Names and addresses of
passengers.
Make of vehi cl e. Any other acti ons requi red by your
stat e.
Model and year of vehi cl e.
