FMEA Homework Assignment 4
Prepare a design or process FMEA on a topic of your choice.
Work as teams if possible and indicate the names of all team members on the form (Max 3 class members on team). Include at least one name and 4 in the file name. If team activity is not possible, work on your own. Be sure to include the names of the in-class team members You may select a generic process such as getting to work on time, etc. Make this as detailed as you wish but (realizing this is the 4thassignment in a 5 wk course), you may keep it simple. The intent is to exercisethe tool, not necessarily to complete an entire analysis. Brainstorm as many potential failure modes as possible and choose at least one PFM to work through RPN.
