HOW TO REQUEST FACILITY, EQUIPMENT AND TOOLING MAINTENANCE
1.0 PURPOSE
The purpose of this procedure is to provide simple method to request work performed by the maintenance department
2.0 SCOPE
This procedure must be used by all employees authorized to request maintenance work on production equipment, facilities and tooling.
The only exceptions are safety issues and emergency equipment/tooling repairs. In those two exceptions VERBAL request is sufficient, with the
understanding that the written request will follow as time allows.
3.0 RESPONSIBILITIES
It is the responsibility of each Manager & Supervisor to fill out, either electronic or hard copy of the Maintenance / Tooling Work Request (Form 501-
2). The Maintenance / Tooling Work Request form is required for any and all Machine / Tool Repairs or Designs and facility issues (excluding IT or
phones).
4.0 DEFINITIONS
Maintenance Work Request Form: document required to request maintenance department action.
5.0 GENERAL
Supervisor level and above are authorized to request maintenance work
Hard Copy or Electronic request are acceptable (blank forms can be downloaded from the BLANK FORMS folder in Projects directory)
No work will be performed without a work request, with noted exception above
6.0 PROCEDURE
(1) Authorized (manager or supervisor) person creates request form & delivers it to the Maintenance Manager in hard copy or e-mail
(2) Maintenance Manager processes request and, if necessary, contacts requesting person for details
(3) Work is then assigned by priority and, if needed, materials are ordered.
(4) Purchasing provides expected arrival dates for materials.
(5) Maintenance Manager schedules work by using available manpower & estimate arrival date of materials.
(6) Work is performed & Maintenance Manager keeps records of time & labor issues.
(7) Upon completion of the job , the requesting person is notified.
