Summary

3
Irrational drug use is widespread and results
in:
Poor patient outcome
High cost of inappropriate drug use
Increased risk of adverse drug reactions
Emergence of antimicrobial resistance

Measuring drug use problems is the first step
to improving use
4
Indicator study methods
Use data collected at the individual level - patient, health facility
Data insufficient to make judgements about appropriateness of a
drug for a specific diagnosis
Aggregate data methods
Use routine data (e.g. stock records) not collected at the
individual patient level
Gives an overview of drug use and can highlight problem
areas
In-depth investigation of drug use
Prescription audit, patient record review
Drug use evaluation (review) (DUE)
Qualitative methods to understand causes of a drug use
problem

The World Health Organizations (WHO)
Essential Drugs List

According to the WHOs definition, essential drugs are
those that meet the healthcare needs of the majority of
the population; thus should be easily available in
adequate quantities and in suitable dosage forms.

13

The National Essential Drugs List

The National Essential Drugs List was formulated by
using the Ministry of Healths (MOH) Drugs List as a
basis. The MOH Drugs List which was introduced in
1983 serves as the essential drugs list for the public
healthcare sector.

Identifies high-priority drugs for selection, procurement
and use in a supply system

- Selection - V & E drugs given priority esp.
when funds are short
- Procurement - Order monitoring
- Safety stock
- Order quantities
- Supplier selection
- Use - Review
- Distribution - Stock control
- Assignment of staff

19

Identifies low-priority drugs to be analyze carefully
for deletion from the formulary

Compared with ABC analysis to monitor how well
actual use compares with priorities

20
Step 1: Classify all drugs on the list as V, E, or N

Step 2: Reconsider proposed purchase quantities

Step 3: Find additional funds if needed or possible

Step 4: Analyze N items. Where possible, eliminate them

Step 5: Analyze N items. Where possible, reduce quantities
to purchase

Step 6: Identify and limit therapeutic duplications

Step 7: Reduce quantities of drugs purchased by using the
preferential weighting or equal misery

What is the
ABC Analysis?
ABC analysis is a business term used to define an inventory categorization
technique often used in materials management.

ABC analysis provides a mechanism for identifying items which will have a
significant impact on overall inventory cost whilst also providing a
mechanism for identifying different categories of stock that will require
different management and controls

When carrying out an ABC analysis, inventory items are valued (item cost
multiplied by quantity issued/consumed in period) with the results then
ranked. The results are then grouped typically into three bands
.
These bands
are called ABC codes.
"A class" inventory will typically contain items that account for 60% of total
value
"B class" inventory will have around 20% of total value
"C class" inventory will account for the remaining 20%

ABC Analysis is similar to Pareto in that the "A class" group will typically
account for a large proportion of the overall value but a small percentage
the overall volume of inventory. - WIPEDIA

Pareto Principle

What is the Pareto Principle?

In 1906, Italian economist Vilfredo Pareto created a mathematical
formula to describe the unequal distribution of wealth in his country,
observing that twenty percent of the people owned eighty percent of
the wealth.

Definition: The Pareto Principle, also known as the 80-20 Rule,
states that a small number of causes (20%) is responsible for a large
percentage (80%) of the effect.

Also Known As: 80-20 Rule, Pareto's Law, Pareto's Principle

General Use: The Pareto Principle is generally applied to vendors or
customers in a retail setting. This rule assumes that 20% of the
customers generate 80% of the sales, or that 80% of merchandise
comes from 20% of the vendors.
31
Category Percentage of
budget
Percentage of
drugs ordered
A drugs 70-80% 10-20%
B drugs 15-20% 10-20%
C drugs 5-10% 60-80%
32
A drugsHigh percentage of funds spent on large
volume or high-cost items
Greatest potential for savings
Greatest potential for identifying expensive drugs that
are overused

B drugsModerate cost and moderate number of
items; important items

C drugsSmall amount of funds spent on the
majority of the inventory
Links consumption to public health needs &
morbidity
Reducing inventory levels & costs
more frequent purchases or deliveries of A items
Cost reduction
seek lower prices for class A items
Assign import and inventory staff
Provide information for:
Choosing most cost-effective alternatives
Therapeutic substitutions
Pharmacoeconomic analysis
Used in combination with VEN analysis
Used as a guide in Drug Committee Meeting
Performing an ABC analysis is facilitated by a
computer and software that will perform the
necessary calculations. A computer is not absolutely
necessary, but it becomes more important as the
number of line items increases and the complexity of
the analysis widens. Management Sciences for
Health provides this software free of charge on its
web site at
www.msh.org
It is easy to download and has instructions on use of
the software in the help section of the main menu.

VEN ANALYSIS

Each member should classify aii the medicine as V, E or N

Result compiled and an overall clasification agreed in the
DCM

DCM identify and limit therapeutic duplication

DCM examine all N items for reduce quantities purchase or
elimination

Reconsider proposed purchase quantities, buying V and E
over N items and ensuring that safety stocks are higher

Monitor drug ordering and stock levels for V and E items
more closely than for N items

ABC ANALYSIS

Pharmacist prepare drug consumption data over a one-year
period or shorter.

Present graphically

Reveal high usage items for which there are lower-cost
alternatives on the list or available in the market
Choose more cost-effective alternative drugs
Identify opportunities for therapeutic substitution
Negotiate lower prices with suppliers

Measure the degree to which actual drug consumption
reflects public health needs
Identify irrational drug use through comparing drug consumption to
morbidity patterns

Identify purchases for items not on hospital essential drug
list

There are numerous methods to obtain information
about drug use.

Aggregate methods like the ABC /VEN analysis are a
useful way to gain an overview of drug use problems
using routine data not collected at the individual
patient level.

We should be aware of these tools to identify drug
use problems and periodically utilize these tools to
assess the current drug use situation.

This close analysis will produce useful information on
drugs and therapeutics that will need more in-depth
evaluation
