Open Google Docs or Google Drive




2. Click Create Select Form






3. Click Theme Select a template


4. Click Apply


5. Name the Form: type in Title in First Box
a. Example: Renaissance Quiz

6. Add Description or Directions in Second Box
a. Example: Please answer each question to the best of your ability and
click Submit.




7. Click arrow in Add item box to add different elements:
a. Examples: questions, page header, page break






8. Mouse over sample question 1 to see three tools:
a. Pencil= editing
b. Boxes= duplicate question
c. Trash= delete item



9. Mouse over sample question, drag & drop it to reorder the questions

10. To email Form:
a. Click email this form
b. Unclick box include form in email
i. Participants may print form and complete in manually instead
of online.





11. Click See responses Select Spreadsheet to see Excel pages
