Program Design Guidelines for Parent-Coordinated Program(s)

1. Program Description
• Purpose and goals of program
• Description of the process used to determine how program will assist consumer in achieving
his/her Individual Program Plan (IPP) objectives
• Anticipated consumer outcomes resulting from participation in the program stated in
measurable terms
• Description of the location(s) in which consumer training occurs
• Schedule of the direct service including a sample of a weekly schedule
• Description of the process used to modify program design
(When modifications are made to the program design which constitutes a change in the type
of services provided, the vendor must, at least 30 days prior to the change, notify the
regional center and submit a revised program design.)

2. Program & Consumer Outcomes / Evaluation


• Evaluation procedures used to determine the extent consumer’s progress toward achieving
the specific outcomes in each IPP objective
(At least annually, vendor shall review the consumer’s performance and progress in relation
to each IPP objective and submit written documentation to the regional center.)

3. Personnel Requirement / Job Description


• Position title
• Role and responsibility of position
• Work hours
• Minimum qualification
• Reporting supervisor

4. Organizational Structure Chart

5. Staff Training
• Staff training plan for new employee orientation and ongoing employee staff training

6. Record Maintenance Procedure


(Records pertaining the program and consumer shall be stored at the program site in a secure and
locked place which shall be readily accessible in the event of an audit.)
