I, Angelique R. Pascua, a Bachelor of Science in Nursing graduate in this institution, batch 2013,
would like to request assistance from your good office in providing me some of the needed
documents for my application on Quebec Skilled Worker Program. The documents needed are
listed below:

Cover letter. Attach herein is a sample of a cover letter. It should contain the university
letter head, address and contact number, the total number of RLE hours, the name of the
university, the start and completion date of the degree in month-day-year format, the date
of issuance and with a university seal.
Internship agreements or attestations from the educational institution stating that the work
internships were performed in the context of studies, with the details as follows
-exact start and end dates of internship
-principal tasks and duties
-number of hours per week
-signature of immediate supervisor or company personnel representative along with his
name written in block letters
-Date of signature
