SRI LANKAN MATHEMATICAL OLYMPIAD / INTERNATIONAL MATHEMATICAL OLYMPIAD 2014


To register for the competition you can either fill in this form or visit our website www.slmathsolympiad.org and fill in the online application form there. If you are registering online visit the website and follow the given instructions. Note that the application fee for online registration is 350/= per student whereas for paper registration it is 400/= per student. If you are using this application please fill this form in English and include students of only one medium in one form. If more forms are needed please photocopy this form. For each student an admission card will be sent to the school address. If you don't receive admission cards on or before April 02, 2014 please call Menuja at 0771137924 or Kasun at 0726711264. Please mail or hand deliver the completed application forms with the deposit slip or a bank-draft obtained after paying the correct amount (Rs.400/= for each student and we do not accept cheques) into the Sampath Bank Account: 1034 6000 0185 (Name of the account is Sri Lanka Olympiad Mathematics Foundation) on or before March 17, 2014 to the address Ms. Menuja Perera, Sri Lanka Olympiad Mathematics Foundation, Department of Mathematics, University of Colombo, Colombo 03. Results will be posted in the web at our website.
