    This form must accompany a First Book category submission. All named authors and publisher must complete this form to be considered eligible for this award category. All writers must be rst-time authors and have not previously authored, co-authored, or signicantly contributed to a food or beverage related book. Should a question arise regarding an entry, the entrant agrees by his/her signature to submit any documentation deemed necessary for review.
