Model Release Form

The photographer took photographs of the model on a specific date at a given location. The model granted the photographer and Impact Imaging usage of the photographs, allowing them to be used wholly or in part in any publication, portfolio, or public display. The photographs could also represent an imaginary person as long as the model's name was not specifically mentioned. By signing the form, the model gave up ownership of the photographs and assigned the copyright to the photographer and Impact Imaging, while the photographs would remain the photographer's property.

I, the model, grant you, the photographer, and all persons within Impact Imaging, usage of the photographs subject to the following conditions: + I understand that the photographs taken of me during this session can be used wholly or in part in any publication (commercial or otherwise), portfolio or public display. + The photographs may be used to represent an imaginary person and any wording associated will not be attributed to me unless my name is specifically mentioned. I acknowledge that by signing this form, subject to the usage restrictions above, I give up all claim of ownership of the photographs, and assign copyright to the photographer, and the persons within Impact Imaging, as mentioned above. Use of the photographs may be granted to third parties; however the photographs will remain the property of the photographer. I have read this form carefully and fully understand the implications.
