Create Forms/Survey/Guestbook for


Your Blog/Websites.

Purpose of this tutorial

Do you have a blog or website or run a newsletter or just want to survey


something by sending your friends a Form using email?

If you answer yes to the above question. Then sometime you would wonder if you
can create a Poll, Survey, Guestbook or any simple form for your website.

Thought there are many websites available for this purpose and they may offer
you free service or paid service.

Many times the free services are limited to number of forms you can create or
number of times a form can be submitted. There could be alternatives that can
offer you free services without any restrictions.

One of such free alternative is using Google Forms.

The Purpose of this presentation/tutorial is to guide you to use Google Forms to


create a interactive form that you can use for your visitors.

This tutorial assumes you have a Google Docs account if you dont have yet you
can go here and create a Free account for your self.

Google Docs

Step 1: Enter Google Doc

• Login to Google Docs


• Click on New Menu and Select Form

Step 2: Create a New Form

• This will open a New Form Window


(If you dont see a new Window
then check if your Browser Popup
Window Blocker is Active then
disable it temporarily)
• Enter Form Title and Brief
description for the purpose of
Form.
• Enter a Question you want to ask.
• Select a Question Type. More on
Question Type on next Step.

Step 2: Question Types

Question Types suggest how do you expect the user

to respond to a particular question .


The Different Types can be explained below:

Text - If you expect just one line reply For Ex: Name
of Visitor or Email Id.

Paragraph Text - Use them if you want the user to


describe something. For Ex: A Feedback Comment, Scale - If you want your user to rate some thing then
Opinion etc. you can specify what scale you want to measure.
For Example If you want to survey how your user
Multiple Choice - If you want the user to select few rate a particular movie you can ask them to rate
options. For Ex: You want to know which search between 1 to 5.
engines your user use and give them options like
Google Yahoo Altavista MSN etc.
You should decide the question type based on what
Check Boxes - Same as Multiple Choice, But here it you want to ask and how you want your user to
will display Check Boxes. respond.

Choose From a List - Select single option from a list


of Choice. For Ex. Yes/No Type of Questions

Step 3: Creating Questions/Form Elements

• After you have framed a Qustion


you can also mark it as Required
Question.
• Click Done once you are done with
this question.
• You can add more questions by
clicking on Add Items as shown in
Figure.

Step 4: Select Theme or Look and Feel

• Once you have framed your Form


now its time for look and Feel.
• Please see the Figure in previous
step and if you select Theme you
will be displayed a screen similar
to what you are seeing in this
Step.
• You can Select a Theme that fits
appropriately to your
website/blog/liking.
• Save the Theme.

Step 5: Ready to use your form.

• Now your form is ready and its time


to use it after you have save id
(Sometimes Google auto saves it
so dont need to worry about this
step.).
• You can select to Send Email to
your friends.
• Or you can select Embed option So
that You can embed a small html
script to your blog post or website.

Step 5: Embed code for your form.

• You can copy the Embed code and


paste it to your Blog posts html
View or any html code.
• Cancel button will close the Embed
code window.
• You can Select See Response
Button to see the responses
recieved from this form.
• Actually Google Docs stores the
Responses for you in form of
Google Spread Sheet.

Step 5: Editing your existing form.

• Editing a existing form can be tricky in google, The reason is as mentioned in previous step that
Google treats form as a special form of Spreadsheet.
• So next time you want to edit your form and you select your document it will open up it as a Spread
sheet containing responses.
• Let it open and If you see the menu item you will see a item labeled Form(Some Number)
• Clicking above will open a menu and you can select Edit Form.
• Though I would not suggest editing an existing form...but sometimes you will need to change it before
any interaction with user has happened.
