Model Direct Submission Form

I, the undersigned Model, knowing that Built Not Bought Magazine, The Octane Media Group, and the photographer are relying thereon, hereby certify as follows: I am the model identified in and who executed this Model Release, Authorization and Agreement (Release). I am at least eighteen (18) years of age (19 years of age if a resident of AL, NE, or WY, 21 if a resident of MS) and am of lawful age to contract and am competent to contract in my own name insofar as this agreement is concerned. I have either been given a license to use and/or full legal ownership of the photos I am submitting by the photographer. I acknowledge and agree that Built Not Bought Magazine and the Octane Media Group has permission to license the use of these same photographs in their publication, to print the images, and to distribute them via their digital channels (facebook, twitter, website) for promotional purposes. I hereby wave any right to inspect or approve said images of me and any advertising copy that would coincide with said images, and I also wave any claim that I may have at any time to the eventual use to which the same may be applied. I hereby release, discharge, and agree to hold harmless, Octane Media Group, Built Not Bought Magazine, and the photographer against any and all liability, claims, actions, and/or obligations as a result of using, editing, or reproduction of the finished product.
