Dear Atty. Maria Carla Rubias,

Please excuse me from being absent last week, October 10, 2013 (Thursday). I was confined to the hospital due to some viral infection and allergies. I have attached my medical certificate as well as my certificate of confinement for further information regarding the physicians diagnosis and the period of confinement.

I am praying for your consideration regarding this matter and allow me to catch up with any class activity I missed during my absence.
