1.1 Finding t h e e n r o l l m e n t form
The enrollment form can be found under the spreadsheet Enrollment_Form, with Table 1 being adjustable to suit client requirements.

1.2 Printing the enrollment form


The enrollment form must be printed and given to interested clients in hard-copy. Printing the enrollment form: 1. File 2. Print 3. Print Settings Print settings: 1. Locate page settings 2. Adjust the scale of the spreadsheet so it fits on an A4 size page.

1.3 Adding client details from a filled enrollment form into the database
Information gathered from the enrollment forms must be manually entered into Table 1 under the spreadsheet classified as Client Contact Info.
