International Applicant Transcript Form

If you are mailing your transcripts or sending your transcripts by courier service please include this form in the package. Your cooperation will ensure more efficient processing of transcripts. We will notify you by email when your transcripts arrive. If the transcripts are sent by courier and signed for in our mailroom you can be assured that they have been delivered. Because the CS department has a very high volume of applicants, it may be not be possible for us to update the Graduate School Application checklist section for received transcripts until after the application deadline. Sending this form with your transcripts will allow us to notify you promptly. It is important that you report your name on this form exactly as it will appear on your Graduate School Application so we can match your transcript to the application. Thank you!
