    1. Demonstrated commitment to a zero incident and injury philosophy. 2. Records of OH&S performance statistics. 3. Records of any fines, citations or improvement notices. 4. Company Change Management Procedures. 5. Compliance/ maintenance checks on plant and equipment. 6. Rehabilitation Procedure 7. Satisfactory References for the past 3 projects. 8. Work Method Statements and/ or procedures for Company nominated high risk activities. 9. Incident reporting & investigation process.

    Example of Evidence Required


    Company brochures, policy statement etc. Previous 12 months and 3 years LTIFR &any fatalities associated with the business. Document details of actual breach and any fines or penalties. Procedures or at a statement on what actions to take if the job changes. Procedure, registers of compliance checks, statement of compliance checking regime. Procedure. Statement of clients. Work Instructions, Procedures or WMS.
