GENERAL SURVEY: Pt. K, 2 years old, female, admitted to PEDIA ward with a chief complaint of fever, seizure and cough. Received patient lying on bed with nasogastric tube attached for administration of fluids and feeding, with IVF of D5IMB 500cc regulated at KVO rate. The patient is unresponsive to stimuli. Dressed appropriately according to weather and gender. VITAL SIGNS: T- 38 C RR- 31 cpm
