STUDENT CLINICAL ROTATION REQUIREMENTS AND PROCEDURES


Submission of the Student Clinical rotation application FORM should be at least 10 days prior the start of your rotation. This Application Form must be completed and submitted to the Department Medical Education along with: 1) The medical school letter should state that your training in HMC is approved. 2) Passport Copy including residency permit page or sponsors Resident Permit. 3) State of Qatar ID copy (if available). 4) Two recent photographs (6X6 with white background) The Request letter should be addressed to: Director of Medical Education Department The Candidate should: Contact Medical Education after 5 working days of submitting the required documents. In case of approval you will be required to come and collect the Approval & ID Badge from Medical Education before the start date of your rotation. You have to report to the concerned Department secretary on the morning of your rotation. Please report any change in your rotation to the Department of Medical Education. Sign the Department attendance sheet. ID Badge should be displayed clearly. An approval from Medical Education is necessary for any extension in the duration of the rotation. Evaluation Sheet should be completed by the supervising Consultant / Trainer at the end of your Rotation.

No Certificate will be issued by Medical Education without: 1. A complete Assessment Form. 2. Returning ID Badge to Medical Education. N.B. Please provide Medical Education with your own evaluation.
