Assignment of Royalties

FOR VALUE RECEIVED, I hereby assign to [Name of Assignee (ie. the Person receiving the
assignment)] all royalties and other moneys, from whatever source received by me under the
terms of a [Describe Agreement giving rise to Royalties (ie. Publishing Agreement)] between me
and [Name of Publisher] dated [Date of Agreement with Publisher (ie. September 22, 2000)] (the
"Publishing Agreement"), regarding the publication of a [Nature of Work giving rise to Royalties
(ie. book, software)] entitled "[Name of Work]", or such other title as may be decided upon.

I hereby authorize [Name of Publisher] to pay directly to [Name of Assignee] all moneys which
would, in the absence of this Assignment, be payable to me under the Publishing Agreement.

Witness [NAME OF GRANTOR]

On the [day] day of [month], [year], before me, the undersigned, a Notary Public in and
for the State of [State of Notary], personally appeared [Name of Grantor], known to me or
proved to me on the basis of satisfactory evidence to be the person who executed the
within instrument, who did execute the within instrument in my presence.
