HIP in Liquidation |
Re: Inquiries Concerning HIP of New Jersey, Inc. d/b/a HIP Health Plan of New Jersey (“HIP”)
Pursuant to the terms of a court order dated June 20, 2008 approving the closure and discharge of HIP, HIP closed its operations on February 28, 2009.
Class 4, 5, & 6 claimants will not receive distributions because of insufficient funds to cover their claims.
Medical Records - Requests for medical records may be made to Iron Mountain, Pennsauken, NJ, at 856-910-8010. Iron Mountain's fax number is 856-486-4468. Medical records were required to be maintained for a 10 year period concluding March 31, 2009. After that date, only infant records will be maintained until the member’s 21st birthday. These records will be destroyed annually as these members reach their 21st birthday.
