I first shared this article several years ago and am revisiting it today. HELP programs have been thoroughly researched and vetted as effective in preventing or lessening the impact of delirium on elderly hospital patients, and yet in my personal and professional experience, I haven’t encountered such programs in area hospitals. As a junior doctor on the ground, I highly recommend watching CNA Talking Point Commentary on the challenges behind prolonged hospital bed wait times in Singapore 🇸🇬. Here are some key takeaways: 🕒 Waiting times: Defined as the time when ED decides to admit a patient to when the patient leaves the ED for the inpatient wards. The median wait time is 7 hours but statistics tell us that this is only one part of the story. 🏥 Supply-demand challenge: Building more hospitals isn't a silver bullet for as long as demand exceeds supply. An ageing population inevitably leads to increased demand for healthcare. An older population carries heavier chronic disease burden leading to longer stays, slowing bed turnover and restricting supply of available inpatient beds. 💡 Decentralization solution: Right-siting care away from hospitals and boosting capacity in home-care, community, and primary care could be key to alleviating strain on hospital resources. It is a sad testament to the state of healthcare in New Yorks State that there are more F rates facilities in New York than anywhere else, and New York does not boast one single A rated facility. I fear the situation will only worsen especially with recent reports that there are more rural hospitals at risk of closure in New York than any other state.
