What a joy to join with Make-A-Wish Middle Tennessee to grant a special wish for 18-year-old Shirley! Following a life-saving liver transplant last year, Shirley’s wish for a mini long-haired dachshund came true today at the Hendersonville, Tennessee Tractor Supply Company. Valentina the puppy weighed only 1.5 pounds, and Shirley received a shopping spree to get everything she needed to care for the pup. We are so happy that Shirley is thriving. She will graduate high school in two weeks and plans to become a nurse. #LifeOutHere #MakeAWish #GivingBack

Today kicks off National Care Leavers Week. 👏 The purpose of this week is to call on the public, professionals, carers, decision-makers, and the media to CARE about care leavers: 🟣 Celebrate care leavers 🟣 Amplify their voices 🟣 Raise awareness of challenges 🟣 Encourage change in policy and practice As a care leaver myself, I know many care leavers do not get the support and advice they need. That's why I’m sharing this video to raise awareness of the challenges that many care leavers face. We often have to combat stigma or preconceptions from people who do not understand our experiences, or assume we are all the same. Learning more about care leavers and the difficulties we can face could help us improve this attitude and achieve positive policy changes. Have you heard that Structured Family Caregiving is coming to Ohio? If you are a caregiver or know someone who is and you are looking for resources, coaching, emotional support and payment, then check out this post from Careforth the national leader in Structured Family Caregiving that clearly defines the program.
Q: What is “Structured Family Caregiving?” A: Structured Family Caregiving (SFC) is a Medicaid-funded benefit that supports primary unpaid caregivers of Medicaid beneficiaries with chronic and complex medical conditions or physical, intellectual, and developmental disabilities to live at home and engage in their communities. An alternative to traditional home care or nursing facility care, SFC allows beneficiaries to receive help with daily care from a family member or friend in their shared home. Currently, Careforth offers Structured Family Caregiving in Connecticut, Georgia, Indiana, Louisiana, Massachusetts, North Carolina, Ohio, Rhode Island, and South Dakota. We also offer Structured Family Caregiving in Pennsylvania through UPMC. More frequently asked questions: https://careforth.com/faq/
