A complex coronary lesion have got multiple definitions- Diffuse long disease, multiple bends, calcification, hemodyanmic instability. what if we find all this in the same vessel? Today did a Angioplasty of elderly male with Unstable Angina. Used multiple size of balloons for serial dilatation followed by cutting balloon for vessel preparation. Distal vessel was badly diseased- accepted the result with POBA. Stent placment in the distal RCA was done with help of guideliner (after failing buddy wire and balloon anchor technique). Happy with the final results and TIMI III flow. Simple coronary lesions are found only in book these days, it seems.
