This is being played out across the nation.  Many issues lead to this: 1)no pcp availability after hours; shortage and limited capacity to modify anything 2) there are no small scale private hospitals run by a group of physicians- this is a very common model in many third world countries. These small level hospitals can deal with many mild to moderate level illness, can admit them, can do moderate level of surgeries. We don’t have anything but ER and main hospitals to go too.  3) lack of trust factor between physicians and patients in current era. Those who are giving answers to patients in ER don’t know them and PCP’s are not involved in any of that as they are busy dealing with their daily work load of entering every single detail in EMR.  4) need modified urgent care centers which has capacity to observe less severe patients for 12-24 hours with capacity to discharge or escalate to main hospitals.  5) give some more legal protections to physicians working in ED and specialist on call who can really triage rather than practicing defensive medicine
