In this nationwide US cohort of patients hospitalized for newly diagnosed for HFrEF, more than 4 of 5 patients were projected as eligible for quadruple therapy at discharge, yet less than 1 in 6 were prescribed it. If clinical trial benefits can be fully realized, in-hospital initiation of quadruple medical therapy for newly diagnosed HFrEF would yield large absolute reductions in mortality. We need to consider any hospitalization for HF as a sentinel event which would require an intensive approach to initiate all 4 pillars of GDMT before hospital discharge, this will increase compliance as well as higher chance on being on these classes of medication at 6-9 months as well as proven early benefits on mortality and HF morbidity.

🔔 And the ball keeps rolling in the early-stage aT1D roller-coaster Until recently, doctors didn't have a standard code to say someone had early-stage autoimmune type 1 diabetes (aT1D). This meant they couldn't bill for helping with this problem or write it down correctly in medical records. 🙌 But thanks to people like Dr. Brigitte Frohnert and researchers from the University of Birmingham and the NHS in the UK, things are changing for the better. Soon, new codes approved by CMS/CDC will help with this. And now, SNOMED has given us a single code to diagnose early-stage aT1D. This code is 1290118005. This is a big deal because SNOMED is used in more than 50 countries around the world, including the US, Canada, the UK, Australia, New Zealand, parts of Asia, and the Middle East. 🎗 Now, people with 2 or more pancreatic autoantibodies but without high blood sugar can be correctly diagnosed as having early-stage aT1D. More #T1D research outcomes reporting from the ADA's diabetes journals that health care providers should know: "Early clinical diagnosis of type 1 diabetes may be beneficial for metabolic control and remission after 3 years of follow-up. Patients diagnosed early may represent a distinct group with better resources or with a different disease biology and slower β-cell destruction, which needs further evaluation." In my experience, hospitalists were much more astute at identifying when a patient was in need of hospice care than other providers involved in the patient's care. Studies have shown that the longer a provider knows a patient, the more likely they are to significantly overestimate prognosis. what can we learn from this to help patient outcomes as well as reducing futile medical treatment. Heart failure (HF) is a complex clinical syndrome that involves abnormalities in the structure or function of the heart, thereby reducing cardiac output and impairing the delivery of blood to metabolizing tissues. Patients with HF present to tertiary care with clinical features such as ankle swelling, shortness of breath and orthopnea. Once heart failure is diagnosed, pharmacological treatment needs to be initiated as soon as possible to improve patient outcomes. The New York Heart Association (NYHA) or World Health Organization (WHO) functional classification systems help in assessing baseline symptom severity and to guide treatment decisions. This audit hopes to encourage healthcare professionals to ensure that patients diagnosed with heart failure receive optimal care. Ample evidence now exists to prove that Guideline Directed Medical Treatment (GDMT) of Heart Failure (HF) reduces the mortality and morbidity of patients with heart failure during hospitalization, prevents their repeated hospitalizations and thus improves their quality of life. https://lnkd.in/eEbrRBFi
