For the past 2 months, I've been lucky enough to work nights and weekends with the incredible team at Afya Research Africa (ARA) on a project to update their marketing and messaging. For rural, low income Kenyans, there is little access to primary health care. Medical care is often far from them and expensive. Since 2009, ARA has been working on this problem, helping thousands of rural Kenyans - especially mothers and children - access primary health care through their network of medical kiosks, or neighborhood storefronts. Unlike private hospitals and public health facilities that don’t serve these communities, ARA’s kiosks are close to home, affordable, staffed with well-trained professionals, and always stocked with critical medicine and supplies. ARA kiosks also treat non-communicable diseases, which are often ignored in these communities. This approach helps improve health outcomes and reduces mortality. Many projects have attempted to fix this "last mile" problem of healthcare delivery, but fail because they leave as soon as they run out of funding. ARA has learned from this and creates social ventures to ensure their own sustainability and continuity of care. This is only one part of the path to scale. As ARA continues to grow, they are partnering with international funders to scale their impact in Kenya. So far, they have partnered with leading organizations like Pfizer, Twinings, UK Aid, and the Kenyan Government. They are now looking for new partners to help bring affordable, quality primary health care to Kenyans. By centralizing administrative functions—such as contracting, payment operations, referral management, and data collection—CCHs enable smaller CBOs to participate in healthcare partnerships, ensuring they can effectively address a wide range of HRSNs. - Efficiency and Effectiveness: CCHs streamline administrative tasks, allowing healthcare organizations to contract with multiple CBOs through a single point of accountability. - Scalability: They provide the infrastructure for scalable and sustainable interventions, leveraging economies of scale. - Improved Outcomes: By coordinating diverse services across a broad geography, CCHs enhance the capacity to address HRSNs and improve health outcomes. The Evolution of Hubs and Their Value Proposition Over the past 15 years, the concept of CCHs has evolved, supported by public and philanthropic investments. CCHs offer a robust value proposition for healthcare entities by enhancing efficiency, ensuring quality service delivery, and fostering trusted relationships with the community. By addressing the social determinants of health, they play a pivotal role in reducing emergency department visits, hospital readmissions, and overall healthcare costs. The Partnership to Align Social Care continues to develop resources to support the creation of efficient, sustainable, and equitable social care ecosystems. By fostering cross-sector collaborations and leveraging the strengths of healthcare organizations and CBOs, we can create a more integrated approach to addressing HRSNs. Engaging with the community can tackle the root causes of health disparities, such as education, housing, and employment. They bring together diverse partners, including community organizations, government agencies, and businesses. "It takes a village to raise a child," 🌱 and I will keep on saying this. This adage isn't just for raising my children but also for building robust healthcare systems. My journey to where I am now is personal. At just 5 years old, I lost my 1-year-old brother due to a lack of timely care – a memory that has deeply shaped my commitment to ensuring first-time-right screening and early diagnosis. While this personal tragedy has been a private burden, it is a reminder of the global reality. Healthcare disparities are real and pressing. The pandemic only worsened them 😷, threatening progress towards universal health coverage. We need innovative models to address human resource gaps and supply chain challenges. Technology and creative solutions are key. 🩺💡 Our journey at Philips Foundation in impact investing is holistic. We assess enterprises for their capacity to generate sustainable impact. Social ventures addressing early screening and diagnosis are vital. Impact investing requires valuing societal impact alongside financial performance. Our commitment to creating positive change for underserved communities remains strong. Although we've made strides as Philips Foundation on the African continent, where I was born and worked for over five years, the need for quality healthcare persists. This need is evident in other regions we focus on as well, such as Latin America and the Indian Subcontinent Recently I’ve visited several events and met with many of our partners – Bayer Foundation, Boehringer Ingelheim, MIT Solve, NLC Health Ventures, UBS Optimus Foundation, We Share Forward Foundation, Business of Impact, Phoenix Capital, Johnson & Johnson Impact Ventures and many more – looking together to invest in smart, intuitive, and compassionate solutions. Does social connectedness matter for health outcomes?     The answer is a resounding YES! The pandemic raised awareness of the negative impact of loneliness and social isolation.     I’m proud to share new research from the US Deloitte Health Equity Institute, led by Elya Papoyan, Nicole Kelm, and Jay Bhatt, that delves into social connections—measured at the family and community levels—as key drivers of health. Among the key learnings:    🏠 Household size and structure vary by income groups, and this impacts health outcomes. Understanding where the gaps exist allows more focused and intentional intervention.    👨👩👦 Partnerships are a form of wealth, particularly for lower-income families. Encouraging healthy and sustainable relationships can help improve community outcomes.    🏞️ Encouraging the use of community spaces may improve overall health and wellness. Talk of people centered healthcare, household inclusion in primary healthcare is very critical. This past week we ZimbosAbantu Healthcare on Wheels , in partnership with the Ministry of Health and Child Care & National AIDS Council, "capped" 60 Community Health Advocates, in one of Zimbabwe's low income informal settlements called Ushewokunze. In case of medical emergency these Advocates are trained to be the first responders while professional medical help is sort, and in these settings it takes quite a while to get medical help, our solar powered mobile clinics come in handy in these hard to reach areas. Peer led interventions at household level are necessary to fight lifestyle diseases such as obesity, diabetes, hypertension etc. ZimbosAbantu Healthcare on Wheels we are playing our part in the fight against NCDs and managing NCDs for those with chronic conditions. Takeda has partnered with the National Urban League to launch a transformative, community-based initiative focused on advancing health equity. This groundbreaking collaboration marks Takeda as the first organization to fund the National Urban League’s health equity infrastructure aimed at addressing the pervasive disparities in healthcare to overcome the barriers that have disproportionately impacted historically marginalized populations. As we all know too well, health inequities result in higher rates of disease and poorer health outcomes among people in racial and ethnic groups when compared to their white counterparts. To solve for these deep-rooted health disparities, we’re committed to collaborating with community-serving organizations that are closest to the nuanced challenges and needs of a population. We believe this will ultimately create lasting change. At the heart of this joint initiative lies the establishment of a Health Equity Continuum of Service Structure across the National Urban League’s network to refer program participants to health care services and increase accessibility within their local communities. This structure aims to bridge the gap between underserved communities and healthcare resources by providing a seamless pathway for program participants to access essential health care. By leveraging the National Urban League’s community presence, this structure will be instrumental in ensuring that healthcare becomes a reality for those who have faced barriers in the past. Additionally, by raising awareness and creating opportunities for continued education, Takeda will support the National Urban League in developing health equity educational webinars and workshops focused on building local capacity around the social determinants of health (SDOH) and the barriers to accessing timely diagnosis and treatments in communities. Long-term efforts under this partnership will engage key stakeholders to shape local and national policy, institutional commitments, and structural investments in communities to achieve health equity holistically. Working alongside trusted leaders in the community and community-based organizations is vital to ensure we’re prioritizing the complex needs of historically marginalized people and can help to implement thoughtful solutions.
