Did you know that safety critical components are one of the main causes of delays in electrical safety testing projects? Generally, a safety critical component is a component which, if changed, could affect the safety of the product. Usually these are mains powered components but can also be secondary components. To meet safety standards, it's crucial that these components are pre-approved to their relevant EN or IEC standard, or UL or CSA standards if submitting for North America. This ensures that each of these components have been tested to their component relevant standards which captures testing and assessment e.g. endurance and loading that would not be assessed under the end product assessment. By reviewing the relevant third-party approval data we can verify that individual components have been tested by a competent (ISO 17025 accredited) test laboratory and they have the particular test standard on their scope of approval. On March 13, 2023, in conjunction with the 19th Global Congress on Process Safety, four of the authors gathered to participate in a panel session. The discussion centered on the premise that many individual device failure rates published by some manufacturers and some certifying agencies are deemed—by members of the panel, many safety instrumented system (SIS) designers, and practicing engineers in the field—to be too low. This is especially true for mechanical final elements. A SIS designed using these unrealistic failure rates often results in estimated safety integrity levels (SILs) for the individual safety instrumented functions (SIFs) comprising the SIS, which would be achievable only under the most optimistic assumptions regarding installation, operation, and maintenance. These estimated SILs are unlikely to be achieved in the field. The panel discussed the underlying causes of this serious safety issue and proposed some possible solutions. This paper summarizes the panel discussion among four of the authors. IEC 61511 and IEC 61508 are both international standards developed by the International Electrotechnical Commission (IEC) related to functional safety, but they have different focuses and applications: 1. IEC 61511: * Title: "Functional safety – Safety instrumented systems for the process industry sector." * Scope: IEC 61511 specifically addresses the functional safety requirements for safety instrumented systems (SIS) used in the process industry sector, including industries such as oil and gas, chemical processing, pharmaceuticals, and power generation. * Focus: The standard provides guidance and requirements for the specification, design, implementation, operation, and maintenance of safety instrumented systems (SIS) to mitigate process-related hazards and risks. * Lifecycle Approach: IEC 61511 adopts a lifecycle approach to functional safety, covering phases such as hazard and risk assessment, safety requirements specification, design and engineering, installation and commissioning, operation and maintenance, and modification and decommissioning. 2. IEC 61508: * Title: "Functional safety of electrical/electronic/programmable electronic safety-related systems." * Scope: IEC 61508 is a generic standard that applies to all industries and sectors and addresses the functional safety of electrical, electronic, and programmable electronic safety-related systems (E/E/PE systems). * Focus: The standard provides a framework and requirements for the development, implementation, and assessment of safety-related systems, irrespective of the industry or application. * Lifecycle Approach: Similar to IEC 61511, IEC 61508 also adopts a lifecycle approach to functional safety, covering phases such as concept, design and implementation, integration and validation, installation and commissioning, operation, maintenance, and decommissioning.
